Early learning centre gastro outbreak notification form — Auckland

Submit this form to notify National Public Health Service of a gastroenteritis outbreak or suspected outbreak at an Auckland early learning centre.

When to use this form

You should notify public health if 3 or more students or staff have gastroenteritis symptoms from a single area or room in your early learning service, Kōhanga Reo or language nest.
